To be eligible for the Loyal Smile Reward, you must:

1. Already be an existing client of Tooth Fairy Dental
2. Book and attend your recall appointment within a month of your due recall set by your dentist and hygienist
3. To continue receiving hygiene loyal Smile Reward you must have had a dental exam by a TFD dentist within the preceding 24 months
4. Comply with requirements of TFD including, but not limited to, any policies regarding payments, outstanding accounts, cancellations or missed appointments
5. Not part of a government subsidised dental scheme like ACC, OHSA, or WINZ

The reward issued:

1. Cannot be redeemed for cash
2. Cannot be used with any other offer or discount
3. Is valid for redemption on hygiene and exam recalls only
4. Cannot be used to purchase products
5. The reward is for the redemption and use of the person attending their recall hygiene or recall exam only and cannot be transferred
6. The reward is only issued if appointment is booked and attended within a month of set hygiene and exam recall
