To be eligible for the Refer & Reward:

1. The new client referred must not have ever previously attended Tooth Fairy Dental and booked in for a Private New Patient Dental Exam (N.B Kids Clubs exams are not eligible for the TFD $40 refer and reward programme)
2. To qualify for the Refer & Reward the referrer recipient must be an existing Tooth Fairy Dental client
3. The recipient cannot be part of a government subsidised dental scheme like OHSA, ACC, OHSA, or under Kids club service fees.
4. Referring clients name must be recorded at the time of booking or attending exam and must be an active patient of TFD.
5. The referred new client must have paid their account in full on the day
6. Comply with requirements of TFD including, but not limited to, any policies regarding payments, outstanding accounts, cancellations or missed appointments
7. Not part of a government subsidised dental scheme like ACC, OHSA, WINZ or CSC2.

The Refer and Reward of $40 credit, to the referred clients account, will be issued the following month. The referrer will be notified when issued.

The Refer and Reward:

1. Cannot be redeemed for cash
2. Cannot be used with any other offer or discount
3. Is valid for redemption on any service at TFD, but cannot be used to purchase products
4. The reward is for the redemption and use of the recipient only and cannot be transferred
5. The reward is only issued if the appointment is booked and attended

At any time, we reserve the right to:

1. Make changes to the Terms and Conditions
2. Terminate, modify or suspend the programme
